Lost Passport
- Please fill the online application through https://embassy.passportindia.gov.in/ and submit the same to Embassy with the following documents:
- Original Police Report along with English translation of the same duly notarized and attested by Ministry of Foreign Affairs, Reykjavik, Iceland. The police report should contain all the details of the lost passport (Number, date and place of issue), name of the applicant, etc.
- An application in plain paper giving reasons for the loss of the passport.
- Affidavit by the applicant explaining the circumstances of the loss of the passport and has not availed of Transfer of Residence facility in the last two years.
- Self-attested photocopy of the previous (lost) passport with first, last, and address pages.
- Copy of Visa page and Residence Permit.
- Photocopy of driving license/identity cards/work permit/stay permit (as applicable).
- In case of documents not in the English language, an unofficial English translation should be attached.
Damaged Passport
- Please fill the online application through https://embassy.passportindia.gov.in/ and submit the same to Embassy with the following documents:
- Duly filled in Passport Application form
- Affidavit-Lost/damaged passport by the applicant explaining the circumstances of damage of the passport.
- Damaged Passport
- Self-attested photocopy of Damaged Passport
- All other documents required for reissue of passport
- The applicants should clearly mention in their applications, reasons for the damage. In case the Consular Officer is satisfied that damage has not been due to the negligence of the applicant, the normal fee might be charged in that case. However, in all other cases, Damaged Passport Fee given in Passport Fee Schedule will be chargeable.
- Misrepresentation of facts may entail the denial of passport facilities. Incomplete applications are liable to be rejected. Issuance of eplacement passport on second loss/damage of passport would be subject to enquiry.
Note : Issue of replacement passport would be subject to Police Verification and receipt of clearances from the authorities concerned in India.
